More common amongst newbie tanker yankers, is forgetting to open the vent(s) before opening the discharge valve! You've only got about 10 seconds or less to to close the valve before the vacuum created in the tank is sucked out enough to keep the manhole covers and vents sucked down, and implodes your $90-120,000 tanker.
Another dandy is opening the discharge too fast or too much when you have a free hose end, like in a manhole. The back pressure in the flex hose can lift the hose out. the same effect as when you pull the trigger on a pressure washer.
Ask a newbie septic pumper driver!

We had a guy forget to vent while pumping on. It cracked and the fluid sprayed out.

My first job driving for an OTR tanker company was hauling leachate (collected septic water from landfills). They used 50 year-old unbaffled tankers left over from their former asphalt business. Most modern tankers come with baffles in them to greatly dampen the shifting liquid as you travel. Ok, so a common tanker has a capacity of 7,000 gallons, which you can fill just about full with gasoline, but leachate water is about 8.4 lb per gallon, Which means you can only haul about 6,000 gallons, or about 50,000 lbs, which leave a lot of room for the leachate to slam back and forth while shifting. Violently! No ABS on those trailers either. a spot opened up in their van ops. and I stayed there 15 years, only occasionally pulling "412s" (DOT 412 chemical trailer, the kind with ribs).
