Can anyone advise what this noise is when pulling off cold?

Only a few of here even have that engine, vast majority here have the 2.3 ecoboost with 10spd auto transmission.

Jeremy as someone else has posted - though this is a forum for all 5g Rangers, the majority of the active members have the USA version of the truck with a different engine and transmission than you have. That said - I'll still try to help.

1) Obvious question; do you have any codes or warning lights?

2) Does this sound happen only when you press the accelerator pedal? In your video that seems to be the case.

3) Are you experiencing a loss of power along with the noise?

In your video this sounds to me like it could potentially be a failing/grinding turbocharger impeller...

I say this because it doesn't seem to be grinding at all times - only when you give it a bit of gas.

Which engine do you have specifically in your truck? It sounds like a diesel.

Thanks for trying to help,

I have no warning lights and no equipment to plug in.

This is the 2.0 twin turbo ecoblue engine. Think it's twin turbo.

It's only when cold for the first 30 seconds. When I put revs on in drive or neutral.

No power lose.

Ford have also given it a clean bill on health ?

Very interesting - does this engine happen to have variable valve timing?

The sound is very reminiscent to a Honda I used to have that needed a new timing actuator. Also rattled on a cold start but only for the first 5 seconds..

If this engine has a timing chain I'd look at guides and other timing / variable timing components.

I have a very similar noise on my 2.3. Doesn’t last nearly as long, but is present any time I feather the gas. I’ve wrote it off as ‘normal’ at this point, but am curious if maybe the cause is the same.

Not sure if the same for your 2.0, but the small rattle sound in this video sounds similar yours is more pronounced.
I would also check all the heat shields to make sure they are secure and not rattling with the exhaust flow also check all the clamps along the exhaust pipe.

The rattle in the video below is more noted when he has the Old / New Turbos (side by side)

To me the sound from your video (sound clip) appears to be exhaust flow related, be it the Turbo's or heat shield or clamp and it is resonating down the exhaust pipe.
