I made a ladder rack out of scrap wood and a half a sheet of plywood awhile ago for just this reason. I've got a 20" ladder I have to move on occasion so it's like way less sketchy to have it hanging over the cab all ratchet strapped in like a madman then having it teetering on the edge of the tailgate.

get some of those ladder hooks, the bigger J-hook kind.

just sheet metal screw one on the passenger front fender, and one on the rear by the taillight. use some bungee cords to secure them into to hooks, and your good to go as long as you dont have a passenger who needs to open the door.
very safe, won't poke anyone.
You laugh but I did something similar with a 20' length of 6" PVC pipe and my old Altima. Used twine and duct tape to attach it to passenger side of car. It stuck out a foot or two at each end but was very secure and didn't clop around. Made loops of twine attached under hood and trunk lid. Duct tape was just insurance.

Did not real all the comments, but here are two suggestions:

1) Open the rear window, pad the lower frame with towels, insert pipes, secure rear with straps.

2) Purchase two 20 foot pipes and tie them under the truck. Laugh all you want, I have done this for years. The largest item I tied was a 6 X 8 X20'. Short run works GREAT!
