I’m in the process of designing my own bed rail to mount bike attachments to. Above the tie downs near the cab, there are two threaded holes on each side, 4 inches apart. Does anyone know the bolt diameter & thread pitch needed for these? Or what the holes are there for from the factory?

FWIW... The bed tie down bolts are 8mm x 1.25 pitch. Maybe remove one of those and see if the holes you are referring to are the same size?

You should have 3 tie downs per side, when FORD was frantically making the ranger they ran out of tie downs, so they started to install only 2 per side. I only had 2 per side; I was able to show my dealer that the trucks should have 3 per side as 90% on the lot had 3. They gave me the additional 2. Think there was a dealer notice to give anyone that asked the additional tie downs.

I think the dealer notice said the opposite, that adding a tie down wouldn't be accepted as a warranty claim.

Even if it did, just by bringing it up usually instills respect that you have done your research and clearly know more then the sales staff. (I had to school the first dealer about several function that i had questions on, I did not buy my truck from that dealership. Even the second one couldn't answer all my questions about some of the functions.) I went back to my sales person and pointed everything out, he just took off 2 tiedowns from one of the other trucks and gave them to me. Granted this leaves the next person with only 4 tiedowns, which i feel bad for, but i got what i wanted and i'll actually use all the tiedowns.

That's generally true if you've done any research at all, because you give more of a shit than the sales guy. That said, this is clearly intentional on Ford's part, so nobody should expect to get 6 tie downs. I think that's stupid, and it would be stupid for a dealership to lose a sale over $30 (or whatever) in parts, but it would also be stupid to walk away from an otherwise good deal over $30 in parts. Basically, no matter how stupid it is, it's not really that important.

Edit to add: since there's some suggestion that the bolts aren't supposed to be reused, I'd rather get a nice new set than have them ripped off some other truck by a junior tech in a hurry, but that's just me.

It was intentional when they ran out of the tiedowns, and they should honor the claims of anyone who only has 4. I see no issue in bringing up that someone should have all 6. $30 in tiedowns may mean little to most people, but for some it is the fact of the matter, which may be the deciding factor.

https://www.ford.com/services/assets/Brochure?make=Ford&model=Ranger&year=2019&postalCode=18657 on page 15 clearly states "Whether you pick the SuperCab with a 6' bed or the SuperCrew®with a 5' bed, you’ll get plenty of cargo space and 6 in-bed tie-downs."

That's true for 2020 Rangers, the brochure doesn't not specify the tie down hook quantities.

But the brochure for the 2019 Ranger specifically states (6) tie down hooks.

That's a disclaimer... there's always a disclaimer. You mentioned a "dealer notice"

I think the dealer notice said the opposite, that adding a tie down wouldn't be accepted as a warranty claim.
