With the icon front and rear coilovers, what does the front come pre-set as out of the box.

I also keep seeing conflicting maximums. Is it 4" max lift or 3.5" or 3"?

Last question, and one of the more important questions, stock UCA's, can I install these at 3.5" with factory UCA's?

I only know the answer to your last question..... With 3.5" of lift, you are going to want to budget in a set of UCA's. I dont think your camber with be within a preferable spec with stock UCA with that much lift. My camber adjustment is pretty much maxed out with only 2" of lift, from Fox 2.0's.
