I could certainly be wrong, but:frown:.......
Looks like the bolt assy used to secure my removable 5000 lb hitch assy, from the carrage frame/hitch receiver box. The expandable section goes inside the hitch box receiver, and when installing the bolt thru the receiver box, prevents the unit from rattling/making noise in the box! I chose one that also "locks" the bolt in place after installation.

I've seen many things in my career picked up in the tread grooves but not quite puncture a tire.
I have too.....but that bolt/washer/spacer looks to "fresh" to have been caught in the tread....no ware from it slapping the pavement.
Even one rotation of a tire with that caught in the tread would give it damage....
