    Maybe your ODB software is configured improperly? Without an upstream O2 sensor nothing is going to work properly in your EFI and Emissions systems. You definitely should have a CEL.

    You are probably right. My truck leaked from the center clip down into #4 even with going through a car wash, decent rain, etc. I fashioned an RTV dam around that center clip and now rain/carwash water is diverted away from the #4 plug. My cover does seem to shield #1-#3 plugs.

    I think what you see on the porcelain of those plugs is called Corona Stain and is normal. I think the plugs actually look fine. They don't appear to be running lean or rich, temp range appears right. I probably wouldn't have changed them, maybe just checked the gap and sent them.

    I can't see any irregular wear, but if you are sure you are getting it after an alignment and rotation, it likely is the tires. It happens from time to time, you get a bad tire(s).
